Combined Skill Checklist Form

FAQ - Frequently Asked Questions

SKILL CHECK

The one page combined skills checklist should be completed, signed and sent to [email protected]. It must be completed to reflect (minimally) oral medication administration and any other skills the student actually performs at work as a DSP (direct support professional).

VERIFY RENEWAL

It is the responsibility of the student and the employer to verify the renewal has been entered into the Ohio DODD MAIS (medication administration information system). The link is on our website and can be found at https://dodd.ohio.gov. Scroll to the bottom of the page and click the link for medication administration. The look-up link is on the lower right side of the page.

RENEWAL

Typically, the renewals are input within a week. If you need it sooner, let us know via email at [email protected]. A copy of the certification is sent to the email used to register for this course.

TIMING

Renewals are annual and can be completed up to 6 months in advance of the expiration date. If the certification has been expired for more than 60 days, the entire Initial 2-day course must be taken again.

ADDITIONAL SKILLS

If the DSP (student) will be performing skills other than what have already been checked during the initial and again upon subsequent renewal certification, the skill(s) must be taught, checked, and documented to keep in employers file. This IST (individual specific training) is required prior to performing any medication administration or health related activities; designed to keep staff informed while keeping individuals safe.
