The Ohio DODD requires that we collect specific information on their application. This ensures that we can enter/update information required to issue your certification renewal.

• Page 1 is for the employer to complete and attest to and sign
• Page 2 is for the student information; (ideally signed by the student when possible) otherwise the employer can complete this and submit it.

Please know that this information is required to enter student data into the Ohio Department of Developmental Disabilities MAIS (meducation administration information system).
